Who is responsible for your information?

Ailsa Witherington, trading as Restore Health Hub, is the data controller for personal information used to provide physiotherapy services.

Information I may collect

When you make an enquiry, book or attend an appointment, I may collect:

• Your name, date of birth and contact details.
• Appointment, payment and correspondence details.
• Relevant health information, symptoms, medical history, assessment findings and treatment notes.
• Details supplied by a referrer or another healthcare professional, where applicable.

How your information is used

I use personal information to respond to enquiries, arrange appointments, provide safe and appropriate physiotherapy care, maintain clinical records, process payments and meet professional, insurance and legal responsibilities.

General personal information is used where necessary to provide your requested service, comply with legal obligations or support the legitimate administration of the practice. Health information receives additional protection and is used for the provision of healthcare and treatment.

Online booking and other websites

Appointments are booked through Cliniko. When you follow a booking link, the information you enter is processed within Cliniko on behalf of the practice and is also subject to Cliniko’s privacy and security information.

This website also links to Instagram. External websites have their own privacy practices, and Restore Health Hub is not responsible for their content or policies.

Sharing your information

Your information is kept confidential. It may be shared only when necessary for your care or practice administration—for example with Cliniko, another healthcare professional involved in your care, a professional adviser or insurer—or where disclosure is required by law or needed to protect someone from serious harm.

I do not sell your personal information.

Keeping information safe and how long it is retained

Reasonable technical and organisational safeguards are used to protect your information. Access is limited to those who need it. Clinical and business records are retained only for as long as required by healthcare, professional, insurance, tax and legal obligations, after which they are securely deleted or destroyed.

Cookies and website data

This website does not currently use advertising cookies, marketing trackers or website analytics. Essential technical information may be processed by the website hosting service to deliver and protect the site. If this changes, this policy will be updated and any required cookie choices will be provided.

Your rights

Depending on the circumstances, you may have the right to ask for access to your information, correct inaccurate information, restrict or object to its use, request deletion, or receive a portable copy. Some information may need to be retained to meet clinical or legal duties.
